What kinds of services do you offer?

We offer comprehensive medical, dental, vision, mental health, acupuncture, chiropractic, pediatric, women’s health and specialty care, as well as wellness groups and support services.

Our triage/advice nurse assists patients in determining if an office visit is necessary. They can also clarify questions about medications and lab results and offer advice to patients per medical protocol.

Please bring the following to your appointment:

  • Co-payment or method of payment (cash, check, debit cards or credit cards are all acceptable forms of payment)
  • Insurance card or evidence of insurance
  • Proof of income if you believe you may qualify for any one of our numerous low-income programs

Please let us know when you arrive if you have had a change in address or telephone number.

We require all medical services to be paid for at the time of service. We accept credit cards, debit cards, cash and checks. We also have a payment plan for those who need to pay for services over several months. Patients can set up recurring payments on a balance due by calling our Billing Department at (707) 559-7677.

You may qualify for financial assistance based on your income level. We can determine this at the time of your visit. Petaluma Health Center provides culturally and linguistically competent care to all members of our community regardless of ability to pay.

Sliding scale is a discount program offered to patients who pay out of pocket that is based on their income levels. A patient with a smaller income will pay less than a patient with a larger income. All patients who qualify for this program should apply for Medi-Cal insurance within 90 days of their appointment. All patients without insurance who are requesting discounted services are required to complete the sliding scale application form. Please print, fill out and bring along to your appointment.
