How do I make an appointment?

You can schedule online through our patient portal or call our main line at (707) 559-7500.

When is the best time to call?

Patients are advised to call during “less popular” hours in order to minimize time spent on hold. Those times are:
11AM – 11:30AM and 2:30PM – 3:30PM

How do I contact a specific department or person?

You can be connected to any department or person by calling our main line at (707) 559-7500 or you can call one of our departments directly.  After hours, the answering service can contact a doctor on call.

How do I refill my prescription?

Please call your pharmacy for refills. If you have questions or are encountering problems, please call your provider.

Where can I receive Medi-Cal dental services?

Petaluma Health Center’s offers dental appointments to Youth (up to 20 years old)  in Petaluma and to all ages in our new  Rohnert Park Dental Clinic.  Both locations accept Medi-Cal.  To schedule a dental appointment in Petaluma, please call (707) 559-7550. For Rohnert Park, please call (707) 559-7602.

My child needs immunizations for school, where and when can this be done?

Please call the appointment desk at (707) 559-7500. No one will be turned away because of inability to pay.

What financial programs do you have, and how do I apply?

There are various financial assistance programs. Please schedule an appointment with one of our enrollment counselors by calling (707) 559-7500 and choosing the ‘Appointment’ option. Petaluma Health Center provides culturally and linguistically competent care to all members of our community regardless of ability to pay.

How can I receive a copy of my medical records?

A request form is available at the reception desk of the Petaluma Health Center. It needs to be filled out and turned in or faxed to the ‘Medical Records Department’. The request will be fulfilled within two weeks and costs $40.

I was not happy with the services I received, whom should I contact?
